Consent to Participate: I confirm my voluntary participation in the Pink October “ Living Well, Living Together”. I acknowledge that I am participating at my own risk and that the organizers are not liable for any injury or loss that may occur during the event. I agree to adhere to all event guidelines and safety instructions provided by the organizers. I further acknowledge and agree that only the organizer Institut Kanser Negara (and not the co-organizer) shall collect the participation fee and process my personal data for the purpose of this Pink October “ Living Well, Living Together" only.*
